Terminal console page can't be closed

Some issues I've seen while reviewing https://github.com/eclipse-platform/eclipse.platform/pull/2418.
Open Terminal page from Console view. 

1) "Close" button is missing, there is no way to close the opened terminal. Closing the Console and re-opening it always reopens the terminal page again.
2) The opened page doesn't have icon (most console pages have one)

I will push a fix in a moment.
